Main Causes of Back Pain

Back pain causes can be broadly categorized into "muscle and ligament problems," "bone and disc problems," and "visceral and other diseases."

Type

Main Conditions

Muscles/Soft Tissues

Myofascial back pain, acute low back pain (lumbago), muscle fatigue

Bones/Discs

Disc herniation, lumbar spinal canal stenosis, compression fracture, lumbar spondylosis

Nerves

Sciatica, piriformis syndrome

Visceral Origin

Urinary stones, pyelonephritis, gynecological diseases, etc.

About 85% of back pain is classified as "non-specific back pain," where no clear cause can be identified through imaging tests. However, there is also "dangerous back pain (red flags)" which may hide fractures, tumors, or infections. Therefore, it is important to seek medical attention early if you experience severe pain or fever.

What is Acute Low Back Pain (Lumbago)?

Lumbago is acute back pain characterized by a sharp pain in the lower back when lifting heavy objects or twisting the body suddenly. Medically, it is called "acute low back pain."

Characteristics of Lumbago

  • Sudden, severe back pain that makes movement impossible

  • Inability to bend forward or stand up

  • Pain is most severe 1-2 days after onset, usually improves within 1-2 weeks

  • Prone to recurrence, so addressing the underlying cause is crucial

What to do immediately if you experience lumbago

  1. Do not force movement; rest in a comfortable position.

  2. If the pain is severe, apply a cold compress to the affected area (acute phase).

  3. Visit an orthopedic clinic as soon as possible to receive pain medication.

  4. Prolonged rest is counterproductive. Start moving little by little once the pain subsides.

About Lumbar Spinal Canal Stenosis and Disc Herniation

Lumbar spinal canal stenosis is common mainly in middle-aged and older individuals, where the spinal canal narrows, compressing nerves and causing back pain, numbness in the lower limbs, and intermittent claudication (leg pain when walking that resolves with rest).

Disc herniation occurs when the nucleus pulposus of an intervertebral disc protrudes and compresses nerves, leading to pain and numbness from the lower back down to the legs. It is also frequently seen in younger individuals.

Both conditions are initially treated with conservative therapy (medication, rehabilitation, nerve blocks). If these treatments are ineffective, patients are referred to a specialized hospital.

Flow of Back Pain Treatment at Our Clinic

  1. Medical interview (when pain started, what movements cause pain, presence of numbness, etc.)

  2. Physical examination (assessment of range of motion, neurological symptoms)

  3. X-ray examination (conducted on the same day if necessary)

  4. Diagnosis and explanation of treatment plan

  5. Prescription (anti-inflammatory pain relievers, muscle relaxants, pain patches, etc.)

  6. Referral to a specialized hospital (for MRI, surgery) if necessary

  • Back pain accompanied by fever

  • Abnormalities in urination or defecation
