Jujo Station Haru Internal Medicine Clinic: Gout & Hyperuricemia Treatment in Jujo
目次
"My big toe suddenly became excruciatingly painful." "My health check-up showed high uric acid levels." Gout is a disease that causes sudden, severe pain, disrupting daily life. Furthermore, even in the asymptomatic stage of hyperuricemia, if left untreated, the risk of kidney damage, urinary stones, and gout attacks increases.
👉 Walk-ins on the day are welcome. Please feel free to consult us first.
What is Gout and Hyperuricemia?
A uric acid level in the blood of 7.0mg/dL or higher is called hyperuricemia. When uric acid accumulates as crystals in the joints, it causes a "gout attack," characterized by sudden, severe pain. The typical symptom is the base of the big toe becoming red and swollen, accompanied by excruciating pain, hence the name gout, as it can be "painful even when the wind blows."
What to do if you have a gout attack?
If an attack occurs, please see an internist as soon as possible.
-
Anti-inflammatory painkillers (NSAIDs) or colchicine are used to suppress inflammation early.
-
During an attack, cool the affected area and rest.
-
Do not start or change uric acid-lowering drugs during an attack (this may prolong the attack).
-
Uric acid level management begins after the attack subsides.
Preventing recurrence is key to managing uric acid levels before an attack, rather than waiting until you are in pain to go to the hospital.
Lifestyle habits to lower uric acid levels
|
Improvement |
Specific measures |
|---|---|
|
Reduce purine intake |
Reduce liver, dried fish, milt, shrimp, etc. |
|
Reduce alcohol intake |
Beer and sake are particularly likely to raise uric acid |
|
Drink plenty of fluids |
More than 2L per day. Promotes uric acid excretion |
|
Reduce fructose intake |
Be careful not to overconsume soft drinks and fruits |
|
Improve obesity |
Weight loss can help lower uric acid levels |
|
Moderate exercise |
Intense anaerobic exercise is counterproductive. Do aerobic exercise. |
Drug therapy
If lifestyle changes alone do not lower uric acid levels, or if attacks recur or kidney function is affected, medication will be used.
|
Type of drug |
Representative drug |
Characteristics |
|---|---|---|
|
Uric acid production inhibitors |
Febuxostat, Allopurinol |
Suppresses uric acid production. Easy to use even for those with poor kidney function. |
|
Uric acid excretion promoters |
Benzbromarone |
Promotes uric acid excretion. Not suitable for those with urinary stones. |
The goal is a uric acid level of 6.0mg/dL or less. Medication is generally required long-term, but if uric acid levels stabilize with lifestyle improvements, dose reduction may be considered.
